Nurse-Led Care
Nurse-led care for seniors at home
Good care starts with understanding the whole person. Our registered nurse visits to learn about your loved one's health, routines, home and what matters most to them.
From that visit, the nurse writes a care plan our caregivers follow, and updates it as needs change. You always have a nurse to talk to, not just a scheduler.

Why a nurse makes the difference
Problems caught early
A trained eye notices small changes in skin, mood, appetite or medication effects before they become emergencies.
Caregivers who are supported
Our caregivers know who to call and what to do. The nurse trains them, checks in and makes sure the care plan is followed.
One plan everyone follows
Family, caregivers and doctors work from the same plan, so nothing falls through the cracks.
Let's talk about what your family needs
Tell us a little about your loved one. We'll listen, answer your questions and help you decide on next steps, with no pressure.
Common questions
Nurse-Led Care: questions families ask
What is a nurse assessment for home care?
A registered nurse visits the home to review your loved one's health, medications, daily routines and safety. The nurse then writes a care plan that our caregivers follow.
What is a delegating nurse?
In Maryland, assisted living programs need a delegating nurse: a registered nurse who assesses residents and oversees the nursing tasks delegated to staff such as CMTs. JTN Health Services provides delegating nurse services to assisted living programs.
Does your nurse work with our doctor?
Yes. Our nurse coordinates with your loved one's doctors and other providers, so everyone works from the same plan.
